Senior Immunizations
Submitted by:

Dear Parents of 11th grade students:

The Ohio Department of Health has made changes to the Ohio Immunization Law requiring all children entering 12th grade to have one or two doses of meningococcal vaccine; depending on when they received their first dose. This requirement went into effect prior to the 2016/17 school year.

Meningococcal vaccine is now State Mandated for preteens at ages 11 or 12 years , with a booster needed between 16-18 years. Meningococcal vaccine protects against meningococcal disease which is a serious bacterial illness. It is the leading cause of bacterial meningitis in children 2-23 years of age. Meningitis is an infection of the covering of the brain and spinal cord.

The Hocking County Health Department offers all immunizations to residents of Hocking County with the following stipulations :
Appointments only. Call 740-385-3030 to schedule
Must bring in child’s immunization record
Must bring in insurance card

You do have other options for your child. Along with the Hocking County Health Department you may choose to receive immunizations through your private physician if they offer immunizations.
Please call their office to make an appointment. Last year all of our local pharmacies offered school required immunizations. Please contact them for further instructions and their policies.

**Logan High School will need to be provided with proof that your child is compliant with the immunization law. Please provide the school with a new copy of your child’s immunization record.** If you have already completed the immunization requirement and sent us the most current record please disregard this letter. You may also fax records to the High School 740-385-9564 or e-mail to

Students will need to have immunizations completed prior to September 8,2017 or they will be EXCLUDED from school until requirements are met.
